Important Info for Feeding Evaluations:

Plan to adjust baby’s schedule so they are ready to eat during the selected session time (if possible). Bring everything you need to feed your baby in all the ways they currently eat (or you would like them to eat- such as prepared bottle, solids if > 6 months). Intake: You will receive intake and consent forms via email upon booking. Please fill out before the night before your session. For home office visits: Our home office is located at 320 Alberta Terrace NE in Buckhead. Please park in front on the street. There will be plenty of easy parking. Evaluation Reports: You have the option to choose a written treatment plan only, or a detailed report with a written treatment plan by selecting the visit type below. You must choose the full report option if you need documentation with detailed information to share with other providers (e.g. ENT, Pediatric Dentist, Gastroenterologist, Tongue tie Release Provider, etc). Sometimes Insurance may ask for this documentation if you choose to submit a superbill. Home Visit Scheduling: In rare cases, appointment times may need to be adjusted due to differences in travel times between home visits. Questions? Email jm@atlantaspeechandfeeding.com with any questions about your appointment or our services. We are looking forward to meeting you and your baby! Thank you!
