Important Info for Feeding Evaluations:

PLEASE NOTE OUR NEW OFFICE LOCATED AT 3133 MAPLE DR Suite 125 ** Plan to adjust baby’s schedule so they are ready to eat during the selected session time (if possible). Bring everything you need to feed your baby in all the ways they currently eat (or you would like them to eat- such as prepared bottle, solids if > 6 months). **Intake: You will receive intake and consent forms via email upon booking. Please fill out before the night before your session. For office visits: There will be plenty of easy parking in front or back of the building. We are located on the first floor and there are no stairs. Evaluation Reports: You have the option to choose a written treatment plan only, or a detailed report with a written treatment plan by selecting the visit type below. You must choose the “full report” option if you have swallowing concerns and/or need documentation with detailed information to share with other providers (e.g. ENT, Pediatric Dentist, Gastroenterologist, Tongue tie Release Provider, etc). Sometimes Insurance may ask for this documentation if you choose to submit a superbill. Home Visit Scheduling: In rare cases, appointment times may need to be adjusted due to differences in travel times between home visits. Questions?
